David L. Falk, LPO, CPO
Mr. Falk earned his Bachelor’s degree at the University of Delaware. He obtained his postgraduate Orthotic certificate degree from Rancho Los Amigos Medical Center in Downey, California. He earned his Prosthetic certificate degree from Northwestern University Medical School at the Rehabilitation Institute of Chicago. Before starting his practice in 1997, Mr. Falk spent many years as a staff practitioner at the Hospital for Special Surgery in New York City where he managed a heavy patient load and gained valuable experience handling complex cases.
Mr. Falk is certified by the American Board for Certification in Orthotics, Prosthetics and Pedorthics (ABC). He is also a member of the American Academy of Orthotics & Prosthetics (AAOP) and the Florida Association of Orthotists & Prosthetists (FAOP). He is a licensed practitioner in the State of Florida as an Orthotist and Prosthetist.
Jeffrey W. Price, LPO, CPO
Mr. Price earned his Bachelor’s degree at Temple University in Philadelphia, Pennsylvania. He obtained his postgraduate Prosthetic & Orthotic certificate degrees from Northwestern University Medical School at the Rehabilitation Institute of Chicago. Before joining Falk Prosthetics & Orthotics, Inc., Mr. Price spent many years as a staff practitioner at Harry J. Lawall & Son, Inc. in Philadelphia, Pennsylvania.
Mr. Price is certified in Orthotics & Prosthetics by the American Board for Certification in Orthotics, Prosthetics and Pedorthics (ABC). He is also a member of the American Academy of Orthotists & Prosthetists (AAOP) and the Florida Association of Orthotics & Prosthetics (FAOP). He is a licensed practitioner in the State of Florida as an Orthotist and Prosthetist.
Justin Kuntze, LPO, CPO, MS
Mr. Kuntze earned his Bachelor’s of Science in Therapeutic Recreation and Park Management from Northwest Missouri State University. He then went on to receive his Masters of Science degree from the University of Utah in Experiential Learning. Mr. Kuntze practiced as a licensed Recreational Therapist for Primary Children’s Medical Center in Salt Lake City helping adolescents with chemical dependency issues and psychiatric disorders. Due to his diverse skills and interests, Justin also began a home remodeling business where he was able to design and build custom additions to homes. When Mr. Kuntze was looking to change careers he wanted to combine his patient care skills and his desire to design and create custom products. Before going back to school Mr. Kuntze volunteered at Shriners Hospital where he gained clinical experience with children. He also worked at Peak Prosthetic Designs as a technician mentoring under Certified Prosthetist, Joe Mahon. Justin had the privilege of helping Joe develop an adjustable prosthetic socket know as the RevoLimb. Mr. Kuntze decided to pursue a career in Prosthetics and Orthotics and attended California State University Dominquez Hills. There he received his postgraduate Certificate degree in Prosthetics & Orthotics. Before joining Falk Prosthetics & Orthotics Inc., Mr. Kuntze practiced at Audubon Orthotic & Prosthetic Services in Colorado treating a wide range of patients in both outpatient and hospital settings.
Mr. Kuntze is a certified by the American Board for Certification in Orthotics, Prosthetics and Pedorthics (ABC). He is also a member of the American Academy of Orthotics & Prosthetics (AAOP) and participates in the mentoring program for new students. Mr. Kuntze is also licensed by the state of Florida and a member of the Florida Association of Orthotists & Prosthetists (FAOP).
Shanon Hardin, LP, CP, MS
Mrs. Hardin earned her Bachelor’s of Science and Masters of Science degrees in Kinesiology from California State University. She earned her post graduate Certificate degree in Prosthetics from Northwestern University Feinberg School of Medicine. Mrs. Hardin completed her prosthetic residency at the VA Medical Center in Long Beach, CA where she has had extensive training in upper and lower limb prosthetics as well as prosthetic rotations at Shriners Hospital for Children, working with children with congenital limb differences. Mrs. Hardin’s expertise also extends to prosthetic gait training with her experience working in a prosthetic gait lab using a combination of kinesiolological and prosthetic factors in assessing prosthetic gait dynamics.
Timothy D. Browning, LO
Mr. Browning earned his Bachelor’s degree at Covenant University in Lookout Mountain, Tennessee. He then earned his postgraduate certificate degree in Orthotics at Shelby State University in Memphis, Tennessee. Mr. Browning is a licensed orthotist by the State of Florida. Mr. Browning has over 30 years of experience in the field of Orthotics & Prosthetics and has an extensive background that includes orthotic instructor at Florida International University where he developed and implemented the orthotic curriculum and served as acting department chairman. He served two terms as president of the Florida Association of Orthotist and Prosthetists (FAOP) and was a facility accreditation examiner for the (ABC).
Cory Hardin, LO, CO, MS
Mr. Hardin earned a dual Bachelor’s and Master’s degree in Kinesiology with an emphasis in athletic training from California State University. He earned his post graduate certificate degree in Orthotics from Northwestern University Feinberg School of Medicine. Mr. Hardin completed his orthotic residency in Riverside, CA where he treated a wide variety of populations ranging from pediatrics to geriatrics. Mr. Hardin’s extensive background in athletic training and biomechanics has enabled him to effectively assess, treat, and provide quality patient care.
Elisa de Jong, LPO, CPO
Elisa de Jong earned her Bachelor’s degree at Eckerd College in St. Petersburg, FL. She obtained her postgraduate Prosthetic & Orthotic certificate degrees from California State University Dominguez Hills. Before joining Falk Prosthetics & Orthotics, Inc., Elisa worked at Mary Free Bed Rehabilitation Hospital in Grand Rapids, MI, and Children’s Memorial Hospital in Chicago, IL. As a staff practitioner she treated a wide range of patients in both outpatient and hospital settings. Elisa is certified by the American Board for Certification in Orthotics, Prosthetics and Pedorthics (ABC) as well as licensed by the State of Florida.
Cyrus Smith, LO, CO
Mr. Smith earned a Bachelor’s degree at the University of South Florida in Tampa, FL with a minor in biomedical physics. He earned his post graduate certificate degree in Orthotics and Prosthetics from Northwestern University Feinberg School of Medicine. Mr. Smith is a Florida native who completed his Orthotics Residency in Tampa where he has extensive training from pediatrics to geriatrics. He is licensed by the American Board for certification in Orthotics as well as licensed by the State of Florida and a member of the Florida Association of Orthotists & Prosthetists (FAOP).
Edward Wells, LO, CO
Mr. Wells earned his Bachelor’s of Business Management from Andrews University in Berrien Springs, Michigan. He then continued to pursue a post graduate Certification in Orthotics from Northwestern University’s Prosthetic and Orthotic Center in Chicago, Illinois. Prior to joining the Falk team of practitioners, Edward had a broad exposure of orthotic experiences working in a level 1 trauma hospital and with numerous rehabilitation centers. Edward has a solid understanding in GAIT and biomechanical principles. He prides himself in providing the best comprehensive care possible while using a very kind mannered approach in patient care. Mr. Wells is certified by the American Board for Certification in Orthotics (ABC) and licensed by the State of Florida as an Orthotist.
Michael Bezzant, LPO, CPO
Mr. Bezzant earned his Bachelor’s degree in Physiology at Brigham Young University prior to completing his Master’s degree in Prosthetics & Orthotics at Alabama State University. His education focused on developing a knowledge of biomechanics and pathologies to guide clinical decision making. He has extensive training and experience working with prosthetic patients. Mr. Bezzant is certified as an Orthotist & Prosthetist by the American Board for Certification in Orthotics, Prosthetics and Pedorthics (ABC) as well as licensed by the state of Florida.
Vicki Thao LO, CO
Mrs. Thao earned her Bachelor’s degree at Metropolitan State University in Saint Paul, Minnesota. She obtained her postgraduate Orthotics and Prosthetic certificate degrees from Century College in White Bear Lake, Minnesota. Mrs. Thao finished her Orthotics and Prosthetics residency in South Florida and treated a wide range of patients from pediatric to geriatric in all care settings. She has extensive training and experience working with pediatric orthoses. Mrs. Thao is certified as an Orthotist by the American Board for Certification in Orthotics, Prosthetics and Pedorthics (ABC) as well as licensed by the state of Florida.
Joshua Stevens
Josh Stevens has been the head technician for Falk Prosthetics & Orthotics for the past seven years. He was born with a congenital anomaly of the left arm, which leaves him without a left hand. Nevertheless, he enjoys helping others and does not allow his amputation to limit his abilities as a technician or in life. Josh is an inspiration to us all as he meets life’s challenges with infectious humor and a smile.
